Admission Forms

We are glad you chose Faithful Shepherd Catholic School for your child’s education! We look forward to these next steps as we welcome you to our community. The following forms are required to be considered for admission:

Completed Application for New Admission
$50 application fee per student
Record Transfer form to Faithful Shepherd office(except kindergarten students – cannot be submitted online)
Copy of Proof of Early Childhood Screening (kindergarten students only)
